Attenuation of Cardiovascular Responses to Endotracheal Intubation with Diltiazem

ABSTRACT
Aim and Objectives: The present study was undertaken to determine the efficacy of diltiazem in attenuation of the cardiovascular response to laryngoscopy and tracheal intubation. Methods: The study included total 50 patients of (age 18- 50 years) both sex with ASA physical status I and MPG grade I & II posted for elective surgery. Patients were randomly divided into two groups of 25 each. Patients who received 0.2 mg/kg diltiazem 60 seconds before the start of laryngoscopy were compared with a control group who received normal saline. Results: Both the groups showed significant rise in mean pulse rate, mean systolic blood pressure and rate pressure product associated with tracheal intubation. Diltiazem group had more increase in pulse rate as compared to control group. The increase in mean systolic blood pressure was statistically more in control group compared to diltiazem group- which showed small margin of rise. However, the rate pressure product was statistically highly significant raised till 5 min in control group as against up to 2 min. in diltiazem groups. Conclusion: We concluded that a single bolus dose of intravenous diltiazem 0.2 mg/kg administered one min prior to laryngoscopy was successful in limiting the acute rise in systolic blood pressure after laryngoscopy and tracheal intubation.
